Muscular Back Pain: Why Your Back May Ache During Everyday Activities

Back pain is one of the most common conditions we see at Spine & Sports Physiotherapy, particularly pain that develops through the muscles on either side of the spine. This type of discomfort is often felt in the upper or middle back and commonly worsens during activities that involve prolonged bending, lifting or standing in a forward position.

For many people, muscular back pain is related to fatigue and reduced endurance in the postural muscles of the back. These muscles work continuously to support the spine and maintain posture throughout the day. When they are required to work harder for extended periods — such as during dishwashing, gardening, cleaning or manual work — they can become overloaded and start to ache or feel tight.

This is similar to how the legs feel tired after walking up a steep hill. The muscles are working hard to maintain the position and may fatigue over time if they are not conditioned for the activity being performed.

The further forward the body leans, the harder the back muscles must work to support the upper body. Carrying, lifting or using the arms while bent forward increases the demand even further. Gradually improving muscular endurance can help the back tolerate these activities more comfortably over time.

Signs of muscular back pain may include:

  • Aching pain beside the spine
  • Pain during bending activities
  • Tightness after prolonged standing or leaning forward
  • Symptoms that ease with rest
  • Fatigue through the upper or middle back

In many cases, slowly building strength and endurance through targeted exercises can help reduce pain and improve tolerance to daily activities. Mild soreness during or after exercise can be normal as the muscles adapt and become stronger.
