Quadriceps Tendinopathy: Using Decline Squats to Strengthen the Knee Tendon
Quadriceps tendinopathy is a common overuse injury that affects the tendon connecting the quadriceps muscles at the front of the thigh to the top of the kneecap. This condition often causes pain around the upper part of the kneecap, particularly during activities such as squatting, jumping, running or going up and down stairs.
Like many tendon-related injuries, quadriceps tendinopathy can become persistent if not managed appropriately. Tendons generally respond best to gradual and progressive loading rather than complete rest. One commonly used exercise for helping strengthen the quadriceps tendon is the decline squat.
Decline squats are performed by elevating the heels using a decline wedge, weight plates, books or a small sloped surface. Raising the heels shifts the body’s weight slightly forward, increasing the demand placed on the quadriceps muscles and tendon during the squat movement.
The exercise involves:
- Standing with the heels elevated
- Performing controlled squats
- Gradually increasing depth and resistance over time
- Progressing load slowly as symptoms improve
Many people begin with bodyweight exercises before gradually adding dumbbells, kettlebells or barbells as strength and tolerance improve.
Before loading exercises, reducing muscle tension through the quadriceps using techniques such as foam rolling may also help improve comfort and movement.
